I agree to adopt the Cat or Dog described above (“the Pet”), and by so doing, accept the terms of this agreement as set forth below. I understand and agree that One Love Animal Rescue, Inc (“One Love”) reserves the right to reclaim the Pet if any of the following conditions are not met, at no charge.*

I agree to care for the Pet in a humane and responsible manner as a member of my family, and to assume full responsibility for the Pet’s well-being for the rest of its life. I agree to provide sufficient shelter for the animal, and to allow the Pet to reside indoors when I am at home, and to sleep inside my home at night (not limited to garage). He/she will not be restricted to the yard, and will not be tied or tethered and left unattended. One Love’s policy is to place animals as family pets only, and not for any other purpose such as guard dogs.*

I agree to provide the Pet with a well-balanced diet, sufficient water and all necessary veterinary care including annual vaccinations (rabies and distemper/DHLP-P) and physical examinations. I understand that the cost of any veterinary care following adoption is entirely my responsibility. I also agree not to debark, crop, dock, declaw or otherwise surgically alter the Pet for any non-medically necessary purpose. I also agree not to transport this dog in an open truck cargo area.*

One Love may reclaim the animal at any time if there is reason to believe that the Pet is being mistreated, abused (verbally or physically), neglected, or is in an unhealthy physical condition. I further understand and agree that One Love representative(s) may make follow-up home visits upon reasonable notice and at a mutually convenient time to verify that all conditions are being satisfied.*

I understand that if I am in breach of the terms of this Agreement, there are costs and damages which will result to One Love that are impossible to ascertain with certainty; therefore, I agree that if I am in breach of this Agreement I will agree to pay to One Love the sum of five hundred dollars ($500.00) as liquidated damages in lieu of proof of actual damages. I agree that this sum is fair and reasonable and is not a fine or a penalty for breach of this Agreement.*

I understand that the Pet is a rescue animal and these pets respond to situations differently and unpredictably. I will provide formal obedience training for my Dog in the event I encounter any behavior problems with the Dog, or in the event One Love requires such training as a condition of adoption. I will submit proof of training from the trainer within 3 months of the adoption date or as otherwise agreed upon by One Love.*

If the adoptee is a dog, a leash will be used when on public streets or in parks or other unfenced areas, and in these areas, you will never allow the dog to roam free unsupervised. Taking time to know your dog will allow you to decide how your dog walks on a leash. Harnesses are recommended by OLAR especially for those dogs that pull and tug or are not easily controlled on leash. Please be aware that using a slip, choke collar, or pinch collar without proper instruction or knowledge can harm your dog’s neck, throat, and trachea. They can also lead to negative association and behavioral issues. The use of electronic shock collars are prohibited to be used on your adopted pet. E-collars are not only inhumane but can limit a dogs true ability to learn and can cause severe behavioral modifications in the future.*

Where applicable, adopter will provide a secure fenced yard and/or ample room for outdoor exercise. You agree to make any necessary repairs prior to allowing Adoptee access to the yard and agree to maintain the security of the fencing now and in the future . As a rescue organization, OLAR questions the safety and efficacy of underground, electric fencing and therefore prohibits the use of them with your adopted pet. Adopter agrees to never leave the adoptee unattended outside for more than play or potty time and to never abandon the animal to fend for itself under any circumstances. Adopter will provide a safe environment including screens on windows (Mainly in the case of a cat adoption). If Adoptee is a kitten, toilet lids will be kept closed.*

I will provide the Pet an identification tag, which includes my name, current address and phone number, and to obtain the necessary licenses for the Pet in the city or county in which I live. I also agree to immediately notify One Love in the event the Pet is lost or stolen.*

It is One Love’s goal to keep all of our pets from ever again being in a kill shelter or abandoned. I agree to contact One Love if the named adopter is not able keep the Pet. If I decide to return my Pet, the Pet will stay in my care as a foster pet until further arrangements are made. I will be bound by the contract of the One Love Foster Home Agreement and expected to partake in activities to facilitate the re-homing of my Pet. This Pet will not be given to or adopted by anyone else without One Love’s prior approval.*

I will call or email One Love within 2 (two) weeks of the adoption date with a report of the Pet’s progress regarding his/her new family and surroundings.*

I agree to take my Pet to a licensed veterinarian within 7 (seven) business days of adoption or no later than _________ (date) for a health check-up and/or vaccinations. Spay/neuter MUST be performed by _________ (date) within 30 days of adoption or upon reaching sexual maturity (4 months of age or as recommended by your veterinarian). Both vet visit and spay/neuter documentation must to sent to One Love via email or mail.*

As per Georgia State Law § 4-13-3, if spay/neuter is not performed in a timely matter, you may be fined up to $200, and the animal may be permanently removed from the home. Violations: It shall be a misdemeanor to fail or refuse to comply with the requirements of Code Section 4-14-3 and any person convicted of said misdemeanor shall be subject to a fine not to exceed $200.00.)*

The donation (not a fee or sale price) for the Pet is $_________ from the adopter to One Love to aid with reimbursement of veterinary, medical and other expenses of the rescuer. No refunds are given if the Pet is returned or repossessed for any reason.*

I hereby authorize ONE LOVE ANIMAL RESCUE (OLAR), a 501 c3 animal rescue organization, based in Savannah , GA, to publish photographs taken of me, (and my name), for use in OLAR’s printed and online publications, website, social media and other related media outlets, as well as for grant application and rescue related donations drives and events. This release applies also to any “story” related to the adopted, fostered animal and its rescue history in the care of the undersigned. I acknowledge that since my participation in publications and websites etc. produced by OLAR is voluntary, I will receive no financial compensation. I release OLAR, its board members, its employees and volunteers from liability for any claims by me or any third party in connection with my participation. I understand that OLAR is a non-profit organization and that use of photographs will be limited to educational, non-commercial purposes. I represent that I am over the age of eighteen and that I have read the foregoing and completely understand the contents thereof. I have read and understand the above:*

IMPORTANT NOTICE - HEALTH & TEMPERAMENT

One Love makes no explicit or implicit guarantees with respect to the health and/or temperament of the Pet. The Pet is adopted “as is” and the adopter assumes all responsibility for treatment of any and all existing conditions or any other physical or temperament changes that may occur. One Love may provide the pet with the basic vaccines. While One Love makes every effort to place healthy animals, we cannot guarantee the health of any animal and shall not be held responsible for any medical or training expenses which may be incurred. I hereby acknowledge the Pet I am adopting on __________ (date) appears to be in good physical condition and does not have any obvious health problems or signs of a contagious illness. I agree that I have received the medical history of this Pet.*

I agree to fully and completely release One Love, its officers, volunteers, agents, employees, contractors and representatives, from any liability of any kind whatsoever arising from any health or temperament issues the Pet may have or may develop, and/or from any and all claims relating to my adoption, ownership or possession of the Pet.*

I agree to indemnify and hold harmless One Love, its officers, volunteers, agents, employees, contractors and representatives from any and all losses, fines, claims or damages, including reasonable attorneys’ fees, arising out of or relating to my adoption, ownership or possession of the Pet, including any claims for injury or damage to persons or property caused by the Pet.*
